Options for Care Management include:

  • Assessing for safety issues at home and providing resources to address concerns with the hopes of aging in place whenever possible

  • Extra support in managing seniors to move to another housing option if needed

  • Overview and connecting to Medicare, Medicaid Caregiving, Palliative Care, Hospice and Long-term Care services when needed

  • Additional support for after a hospital event

  • Support with Advance Care Planning and/or choosing a Healthcare Decision Maker, if needed

  • Extra support in attending medical appointments, hospital visits and acting as proxy for your aging parents when you need to take a vacation (limits: I cannot make healthcare decisions as I am not the Healthcare Power of Attorney, but I can communicate with the POA and facilitate having them give permission by phone or in writing to the medical entity involved)

  • Informal caregiver support that is not medical and not hands on.

  • Care Management Services can be customized to fit your needs. Depending on how much support is needed, there may be an hourly rate or a flat fee. Initially, there will be an extensive 2 hour assessment at $150/hr which may be reimbursed by Long Term Care**

We are a current Certified Care Manager through the National Academy of Certified Care Managers to help plan these complex situations.

**Medicare/Medicaid/Insurance does not typically pay for these services. They are private pay. However, if you have Long-Term Care Insurance, you will likely have the initial assessment covered or reimbursed as part of your person’s LTC policy (check your policy).
